Mast cells are an important part of the immune system, but when they don’t work properly, they can cause a wide range of symptoms that affect daily life. Understanding how mast cells function and what happens in mast cell diseases is an important step in managing your health and finding the right support.

Mast cells (MC) are immune system cells that live in the bone marrow and in body tissues, internal and external, such as the gastrointestinal tract, the lining of the airway, and the skin. Everyone has mast cells in their body, and they play many complex and critical roles in keeping us healthy. The positive roles that they play include protecting us from infection, and helping our body by participating in the inflammatory process. However, mast cells are also involved in allergic reactions, from the tiny swelling that appears after a mosquito bite to a life threatening, full-blown anaphylaxis.

Mast cells have within them small sacs, or granules, surrounded by membranes. The sacs contain many different kinds of substances called mediators, which participate in all of the roles above, including allergic response and anaphylaxis.

The mediators are selectively released when there is an allergic or mast cell based reaction.1

There is a difference between someone who is healthy, with mast cells that are functioning normally, and someone with a mast cell disease, whose mast cells may be activating inappropriately in response to triggers, or may also be proliferating and accumulating in organ tissues.

What are Mast Cell Disorders?

Mast cell diseases are caused by the proliferation and accumulation of genetically altered mast cells and/or the inappropriate release of mast cell mediators, creating symptoms in multiple organ systems.2 The three major forms of mast cell diseases are mastocytosis,  mast cell activation syndrome (MCAS), and Hereditary Alpha tryptasemia (HAT). Mast cell diseases can cause tremendous suffering and disability due to symptomatology from daily mast cell mediator release, and/or symptoms arising from infiltration and accumulation of mast cells in major organ systems. Although systemic mastocytosis is a rare disease,3 those suffering with MCAS have recently been increasingly recognized and diagnosed. As a result, patients with MCAS appear to represent a growing proportion of the mast cell disease patient population.4, 5 It is important to note that the process of mast cell activation can occur in anyone, even without a mast cell disease, as well as in patients with both mastocytosis and MCAS.6

Mastocytosis

Definition

Mastocytosis has been defined in the literature as an abnormal accumulation of mast cells in one or more organ systems. Previously classified by the World Health Organization (WHO) as a myeloproliferative neoplasm, mastocytosis is now classified in its own category under myeloid neoplasms.7 Broadly separated into three categories, cutaneous mastocytosis (CM), systemic mastocytosis (SM) and mast cell sarcoma these diseases occur in both children and adults. CM is considered a benign skin disease representing the majority of pediatric cases. In 67-80% of pediatric cases seen, resolution will occur before or in early adulthood.8-10 In pediatric mastocytosis, symptoms of mast cell mediator release may occur systemically as a result of mast cell mediators released from skin lesions.10 This, however, does not necessarily indicate systemic disease. The incidence of systemic pediatric disease was previously unknown, but systemic forms have now been proven to exist in some children.8-10 The majority of adult patients are diagnosed with systemic disease. Skin involvement, typically maculopapular cutaneous mastocytosis/urticaria pigmentosa, is common in adult patients and can provide an important clue to accurate diagnosis.11, 12

Diagnosis and Classification13-17

CM is diagnosed by the presence of typical skin lesions and a positive skin biopsy demonstrating characteristic clusters of mast cells. The preferred method of diagnosing SM is via bone marrow (BM) biopsy. The WHO has established criteria for diagnosing SM, summarized18 as follows:

Major ª: Multifocal dense infiltrates of mast cells (MCs) (> 15 MCs in aggregate) in tryptase stained biopsy sections of the bone marrow or other extracutaneous organ

Minorª:

  • More than 25% of MCs in bone marrow or other extracutaneous organ(s) show abnormal morphology (i.e. are atypical MC type 1 or are spindle–shaped MCs) in multifocal lesions in histologic examination
  • KIT mutation at codon 816b in extracutaneous organ(s) (in most cases bone marrow cells are examined)
  • KIT+MCs in bone marrow show aberrant expression of CD2 and/or CD25
  • Serum total tryptase > 20 ng/mL (does not count in patients who have AHN-type disease.)

Abbreviation Key:

ª If at least one major criterion and one minor criterion OR at least three minor criteria are fulfilled, the diagnosis of systemic mastocytosis can be established.

Mast Cell Activation Syndromes

Definition

Existence of a subset of mast cell disease patients who experience episodes of mast cell activation without detectable evidence of a proliferative mast cell disease was postulated over 20 years ago.19, 20 Over the last two decades, with development of improved methodology for identification of abnormal mast cells,21-24 it became apparent that there were patients who exhibited symptoms of mast cell mediator release who did not fulfill the criteria for SM.25, 26 Thus began the evolution of discussions about other forms of mast cell diseases, both clonal and nonclonal, which became known as Mast Cell Activation Syndromes (MCAS).6, 27, 28

Diagnosis and Proposed Classification

Recognition by specialist physicians of the importance of mast cell activation in disease led to an international Mast Cell Disorders Working Conference emphasizing this topic in September of 2010. Consensus statements were published regarding classification of and diagnostic criteria for mast cell diseases,6 where mast cell activation plays a prominent role.

Mediators produced by mast cells have a considerable effect on specific symptomatology. Symptoms, including, but not limited to flushing, pruritis (itching), urticaria (hives), headache, gastrointestinal symptoms (including diarrhea, nausea, vomiting, abdominal pain, bloating, gastroesophageal reflux), and hypotension (low blood pressure), allow a patient to meet the first of three required co-criterion for systemic mast cell activation when the patient exhibits symptoms involving two or more organ systems in parallel, which recur, or are chronic, are found not to be caused by any other condition or disorder other than mast cell activation, and require treatment or therapy.6, 28

The second required co-criterion for systemic mast cell activation depends on documentation that mast cells are directly involved in the symptomatology. An increase in the serum level of tryptase, above baseline and within a narrow (generally accepted as one to two hour) window of time after a symptomatic episode, is proposed as the preferred method for providing evidence of mast cell involvement according to these criteria.6, 28-30 The consensus article provides a method for calculating the required minimum rise in serum tryptase.6 After a reaction, a level of serum tryptase that is a minimum of 20% above the basal serum tryptase level, plus 2 ng/ml, will meet the second criterion listed above for a mast cell activation event (see Tests for further information). Consensus members also agreed that when serum tryptase evaluation is not available or when the tryptase level does not rise sufficiently to meet the required increase for the co-criterion, other mediator tests could suffice. A rise in urinary n-methyl histamine, prostaglandin-D2, or its metabolite, 11β-prostaglandin-F (24-hour urine test for any of the three), is considered an alternative for the co-criterion related to a requirement for a mast cell mediator level rise during a systemic mast cell activation event.6

Finally, the third co-criterion requires a response (based on response criteria15) to medications that inhibit the action of histamine.6 In addition, in those with typical mast cell activation symptoms, a “complete or major” response to drugs that inhibit other mediators produced by mast cells or block mast cell mediator release can be regarded as fulfillment of the third co-criterion for MCAS.6, 28

Cílem této webové stránky je poskytnout pacientům z různých zemí prostor ke sdílení a vyhledávání informací o mastócytóze.

Tento projekt si klade za cíl poskytovat nejen obecné informace o tomto onemocnění, ale také kontakty na jednotlivé podpůrné spolky pro masto-pacienty z různých zemí za účelem usnadnění jejich vzájemné komunikace.

Žírné buňky – Mastocyty

Žírné buňky jsou součástí imunitního systému. Jsou produkovány kostní dření odkud putují do tkání, kde se usazují poblíž cév nebo nervů. Za normálních okolností se nenacházejí v periferní krvi, proto lze přítomnost žírných buněk zjistit výhradně biopsií tkáně (např. kůže nebo kostní dřeně), ale ne ze vzorku krve.

Žírné buňky produkují a uchovávají mediátory (např. histamin), které se z nich pak mohou uvolňovat, což následně vede k projevům symptomu.

Mastocytóza

Jak již bylo řečeno, mastocyty (žírné buňky) jsou součástí imunitního systému každého člověka. Rozdíl u pacientů trpících mastocytózou je abnormální růst a hromadění žírných buněk v jednom nebo více orgánových systémech (např. kůže, kostní dřeň, játra, slezina).

Mastocytóza je onemocnění, které se může projevovat mnoha různými způsoby.

Symptomy

Symptomy jsou často způsobeny degranulací nebo uvolněním (vylitím) mediátorů z žírných buněk.

Žírné buňky obsahují granule mediátorů včetně histaminu, které mohou být uvolněny ven z žírné buňky. Toto uvolnění mediátorů může vést k projevu symptomů jako je flush (červenání), svědění, průjem, špatné trávení, nevolnost, břišní bolesti, svalová bolest a bolest kostí, osteoporóza, změny krevního tlaku (obvykle pokles krevního tlaku – hypotenze), anafylaxe, zrychlený tep, bolest hlavy, problémy s koncentrací, únava, závratě, podrážděnost, deprese atd.

U každého typu mastocytózy mohou být symptomy akutní nebo chronické. Síla symptomů může být mírná nebo život ohrožující a může záviset na spouštěčích, které se mohou výrazně lišit u jednotlivých pacientů.

Světová zdravotnická organizace WHO – klasifikace mastocytózy

Kožní mastocytóza (Cutaneous Mastocytosis, CM)

  • Makulopapulózní kožní mastocytóza (MPCM), dříve: Urticaria Pigmentosa (UP)
  • Difuzní kožní mastocytóza ( DCM)
  • Mastocytom

Systémová mastocytóza (Systemic mastocytosis, SM)

  • Indolentní systémová mastocytóza (nejčastější forma systémové mastocytózy)
    • Izolovaná mastocytóza kostní dřeně (BMM)
    • Doutnající systémová mastocytóza (SSM)
  • Systémová mastocytóza spojená s klonální hematologickou nemocí z jiných buněk než mastocytů (SM-AHNMD)
  • Agresivní systémová mastocytóza (ASM)
  • Leukémie žírných buněk (MCL)
  • Sarkom z žírných buněk (Mastocytový sarkom)
  • Extrakutánní mastocytom

Světová zdravotnická organizace rozděluje mastocytózu na dvě různá onemocnění: kožní mastocytózasystémová mastocytóza.

1. Kožní mastocytóza (CM), což je výhradně dermatologické onemocnění, ačkoli symptomy mohou být systémové.

U kožní mastocytózy je nahromadění žírných buněk izolováno v kůži. Jde VŽDY o benigní (nezhoubné) onemocnění.
Kožní mastocytóza postihuje především děti a u většiny z nich odeznívá okolo puberty.
Ve většině případů není kožní mastocytóza dědičná.

a) typy kožní mastocytózy

Makulopapulózní kožní mastocytóza (MPCM), dříve: Urticaria pigmentosa (UP) je nejčastějším typem kožní mastocytózy. Projevuje se mnohočetnými malými růžovými nebo hnědými skvrnami, popř. zduřeninami na těle, obvykle však ne v obličeji.

Telangiectasia macularis eruptiva perstans (TMEP) je vzácná druhotná forma MPCM. Projevuje se jako přetrvávající růžové skvrny na kůži těla u dospělých pacientů. Můžeme pozorovat také flush (červenání) a svědění.

Difuzní kožní mastocytóza (DCM): velmi vzácná forma projevující se zhrubnutím kůže a snadno se tvořícími puchýři.

Mastocytom: druhá nejvzácnější forma vyskytující se u kojenců a batolat. Může se projevovat jedním nebo mnohočetnými vystouplými uzlinkami.

b) kritéria pro diagnózu kožní mastocytózy

Diagnóza kožní mastocytózy je založena na klinickém a histologickém rozboru kožních ložisek a absenci jednoznačných projevů systémového typu mastocytózy, což je:

  • typický exantém, Dariérův příznak
  • biopsie kůže s pozitivním nálezem:
    • mastocytární infiltrát (nad 15 buněk) neborozptýlené žírné buňky (více než 20 buněk)
    • zjištění mutace c-KIT

2. Systémová mastocytóza (SM) je hematologické onemocnění

Systémová mastocytóza je myeloproliferativní neoplasma. K množení žírných buněk dochází nejméně v jednom orgánu (obvykle v kostní dřeni) a musí být potvrzeno biopsií.

Kožní projevy mohou, ale také nemusí být přítomny. Systémová mastocytóza se obvykle objevuje u dospělých pacientů. Má chronický průběh a je nevyléčitelná.

Prognózy jsou u systémové mastocytózy obvykle příznivé. Ve vzácných případech může jedna forma systémové mastocytózy přejít v jinou. Ve většině případů není systémová mastocytóza dědičná.

a) typy systémové mastocytózy

Indolentní systémová mystocytóza (ISM): je nejčastějším typem systémové mastocytózy. Může zůstat beze změny po celý život nebo ve vzácných případech přejde do jiného typu mastocytózy. Jedním jejím druhotným typem je Dřeňová mastocytóza, která není nikdy provázena kožními projevy. Druhým typem jeDoutnající systémová mastocytóza, která vykazuje vyšší zatížení žírných buněk.

Systémová mastocytóza s klonálním hematologickým nemastocytárním onemocněním (SM-AHNMD): několik typů krevních onemocnění. Mohou být zjištěny běžným krevním testem nebo biopsií kostní dřeně.

Agresivní systémová mastocytóza (ASM): toto onemocnění je provázeno poškozením minimálně jednoho orgánu. Kožní projevy jsou u tohoto typu vzácné.

Mastocytární leukémie (MCL): jedná se o extrémně vzácné hematologické onemocnění. Kožní projevy se u tohoto typu nikdy nevyskytují.

Existují dva velmi vzácné typy nádorů z žírných buněk. Mastocytový sarkom, který je maligní a Extrakutánní mastocytom, který je benigní a vyskytuje se na různých místech v těle, ale ne na kůži.

b) kritéria pro diagnózu systémové mastocytózy

Diagnóza systémové mastocytózy může být stanovena u pacientů jejichž biopsie vnitřního orgánu (obvykle kostní dřeně) splňuje alespoň jedno hlavní nebo nejméně tři vedlejší kritéria.

Hlavní kritérium

Multifokální, densní mastocytární infiltráty (nad 15 buněk)

Vedlejší kritéria

  1. více než 25% žírných buněk je atypických (vřetenobuněčných nebo protáhlých)
  2. průkaz bodové mutace v kodonu 816 genu KIT
  3. mastocyty s koexpresí CD2 anebo CD25
  4. celková tryptáza v séru nad 20 ng/ml

c) další vyšetření zahrnují: celkový krevní obraz, ultrazvuk, DEXA scan, endoskopické vyšetření.

Léčba

Léčba indolentní systémové mastocytózy je zaměřena na symptomy a spočívá ve vyhýbání se spouštěčům, které mohou způsobit degranulaci žírných buněk. K léčbě se používají antagonisté histaminových receptorů (antihistaminika). U agresivnější formy systémové mastocytózy se užívá specifická cytostatická léčba (chemoterapie). Zřídka je nezbytná také polychemoterapie, ozařování, nebo transplantace kmenových buněk.

Introductie

Deze website biedt patiënten van verschillende landen een gezamenlijk platform om informatie over mastocytose te zoeken en met elkaar te delen.

Het doel is om een algemene introductie te geven over de ziekte en om links te bieden naar verschillende Europese patiëntenverenigingen voor het ondersteunen van onderlinge contacten en informatie-uitwisseling.

Mestcellen

Mestcellen zijn onderdeel van ons immuunsysteem. Ze worden aangemaakt in het beenmerg en verplaatsen zich van daaruit naar weefsels waar ze zich ophouden in de buurt van bloedvaten of zenuwen. Mestcellen worden over het algemeen niet gevonden in het bloed, daarom kan de aanwezigheid van mestcellen alleen worden vastgesteld door middel van een biopsie (bv. in huid of beenmerg) en niet in een bloedmonster. In de mestcellen worden stoffen geproduceerd en opgeslagen (bv. histamine); als deze vrijkomen kan dit leiden tot symptomen.

Mastocytose

Zoals hiervoor al is gezegd zijn mestcellen onderdeel van ieders immuunsysteem, dus ook bij gezonde mensen. Het verschil is dat er bij patiënten met mastocytose een abnormale groei en accumulatie (vermenigvuldiging) van mestcellen in één of meerdere organen (bv. huid, beenmerg, lever, milt) is. Mastocytose is een aandoening die zich in verschillende vormen kan uiten.

Symptomen

Symptomen worden vaak veroorzaakt door degranulatie (vrijkomen) van mediators (hulpstoffen) die zijn opgeslagen in mestcellen. Mestcellen bevatten granules (pakketjes) van chemicaliën zoals histamine die uit de mestcel kunnen vrijkomen. Deze chemische uitstoot kan leiden tot verschillende symptomen: flushing (rood worden), jeuk, diarree, maag- en darmstoornissen, misselijkheid, buikpijn, spierpijn, botpijn, osteoporose (botontkalking), hypotensie (verlaagde bloeddruk), anaphylaxie (bewustzijnsverlies), hartkloppingen, hoofdpijn, concentratieproblemen, extreme vermoeidheid en uitputting, brainfog (sufheid), duizeligheid, irritatie, boosheid, depressie en angst. Er zijn verschillende typen mastocytose en de symptomen kunnen acuut of chronisch zijn. De hevigheid van de symptomen kan mild zijn maar ook ernstig, een acute anaphylaxie is potentieel levensbedreigend, en is afhankelijk van triggers (prikkels) die enorm kunnen verschillen tussen patiënten onderling. In het zeldzame geval van agressieve systemische mastocytose en in mestcel leukemie kunnen symptomen optreden door de verandering en vernietiging van organen door mestcelinfiltratie.

  • Maculopapular cutane mastocytose (MPCM) (ook genoemd Urticaria Pigmentosa (UP))
  • Diffuse cutane mastocytose (DCM)
  • Mastocytoma

Systemische Mastocytose (SM)

  • Indolente systemische mastocytose (ISM) (meest voorkomende vorm van SM)
    • Geisoleerde beenmerg mastocytose (BMM)
    • Smeulende systemische mastocytose (SSM)
  • Systemische mastocytose met een daaraan geassocieerde bijkomende clonale hematologische ziekte (SM-AHNMD)

De Wereld Gezondheidsorganisatie (WHO) heeft mastocytose onderverdeeld in twee verschillende ziektes: cutane mastocytose en systemische mastocytose.

1. Cutane mastocytose (CM), die een exclusief dermatologische ziekteis, hoewel de symptomen veroorzaakt door de mediators systemisch van aard kunnen zijn.

Bij cutane mastocytose is er alleen in de huid een toename van mestcellen. Het is ALTIJD een goedaardige ziekte. Cutane mastocytose wordt vooral gevonden bij kinderen en verdwijnt bij de meeste kinderen in de puberteit. Meestal is cutane mastocytose niet erfelijk.

a) Verschillende typen cutane mastocytose

Maculopapulare cutane mastocytose (MCPM), voorheen genoemd urticaria pigmentosa (UP) is het meest voorkomende patroon. Het bestaat uit vele kleine roze of bruine vlekken of zwellingen die op het lichaam verschijnen maar meestal niet in het gezicht. Telangiectasia macularis eruptiva perstans (TMEP) is een zeldzame subvariant van MPCM. Het geeft blijvende roze vlekken op de huid van het lichaam van volwassenen. Het geeft flushing en jeuk rond de kleine bloedvaten.

Diffuse cutane mastocytose (DCM): een erg zeldzame vorm die aanwezig is bij de geboorte; de huid is verdikt en er ontstaan vaak blaren.

Mastocytoma: een zeldzame vorm die wordt gezien bij kinderen. Het geeft enkelvoudig of meervoudig verhoogde knobbels te zien.

b) Diagnostische criteria voor cutane mastocytose

De diagnose van cutane mastocytose is gebaseerd op onderzoek naar klinische (het ziektebeeld) en histologische (in de weefsels) lesies (beschadigingen) van de huid en de afwezigheid van definitieve criteria van systemische betrokkenheid:

  • typisch eczeemachtig; het teken van Darier (het zwellen en opvlammen van de huid als je erover wrijft) kan positief zijn.
  • positieve huid biopsie:
    • mestcelinfiltratie in de huid (> 15 mestcellen) of verspreide mestcellen (> 20 mestcellen)
    • opsporen van een zg. KIT-mutatie in de mestcellen.

2. Systemische mastocytose is een hematologische ziekte

Systemische mastocytose is een zogenaamd myeloproliferatief (het beenmerg maakt te veel van een bepaald type bloedcellen aan) neoplasme (vorming van nieuw weefsel). Mestcelproliferatie wordt gevonden in minstens één orgaan (meestal het beenmerg) en moet worden bevestigd door een biopsie. De huid kan betrokken zijn maar dat hoeft niet. Systemische mastocytose doet zich normaal voor bij volwassenen. Het heeft een chronisch verloop en kan niet worden genezen. De prognose van systemische mastocytose is meestal gunstig. Heel soms verandert een vorm van mastocytose in een andere vorm. In de meeste gevallen is systemische mastocytose niet erfelijk.

a) Verschillende typen systemische mastocytose

Indolente systemische mastocytose (ISM): de meest voorkomende vorm. Het kan onveranderd blijven door het leven heen en verandert zeldzaam in een ander type. Een subvariant is beenmerg mastocytose, die nooit samengaat met huidaandoeningen. Een andere subvariant is smeulende systemische mastocytose met veel meer mestcellen.

Systemische mastocytose geassocieerd met een bloedaandoening (SM-AHNMD): bij sommige patiënten kunnen zich verschillende typen bloedziekten ontwikkelen. Ze kunnen worden gevonden door een routine bloedtest of een beenmerg biopsie.

Agressieve systemische mastocytose (ASM): deze ziekte gaat samen met de beschadiging van tenminste één orgaan. Huidaandoeningen zijn zeldzaam.

Mestcel leukemie (MCL): dit is een buitengewoon zeldzame hematologische (bloed) ziekte. Er zijn geen huidaandoeningen.

Er zijn twee verschillende en zeer zeldzame mestcel tumoren. Mestcel sarcoma, die kwaadaardig is en extracutane mastocytoma, een goedaardige tumor die zich niet in de huid bevindt.

b) Diagnostische criteria voor systemische mastocytose

De diagnose van systemische mastocytose wordt vastgesteld bij patiënten wiens biopsie van een inwendig orgaan (meestal het beenmerg) voldoet aan ten minste één major (groot) criterium en aan ten minste drie minor (kleine) criteria.

Major criterium

Multifocale (op meerdere plaatsen) dichte infiltratie van mestcellen (> 15 mestcellen per ophoping)

Minor criterium

  1. meer dan 25% van de mestcellen laten een abnormale vorm zien (kluwenvormig of langwerpig)
  2. ontdekking van een genetische afwijking van de mestcellen, de KIT-mutatie op codon 816
  3. mestcellen laten CD2 en/of CD25 zien
  4. serumtryptase (een specifiek eiwit voor mestcelactiviteit in het bloed) > 20 ng/ml

c) Verder onderzoek kan bestaan uit een volledige telling van bloedcellen, echo-onderzoek, botdichtheidscan en endoscopieën (inwendig cameraonderzoek).

Behandeling

De behandeling van indolente systemische mastocytose is symptoom georiënteerd en bestaat uit het vermijden van individuele triggers die mestcel degranulatie kunnen veroorzaken. De behandeling bestaat primair uit het toedienen van anti-histamines. Meer agressieve vormen van systemische mastocytose worden behandeld met cytoreducerende medicatie (chemotherapie). Heel soms is meervoudige chemotherapie, bestraling of beenmergtransplantatie noodzakelijk.

Introduction

Le but de ce site est d’offrir aux patients de différents pays une plate-forme commune leur permettant de rechercher et de partager des informations sur la mastocytose. L’objectif est de fournir une introduction générale sur la maladie ainsi que des liens vers divers sites européens de soutien aux patients afin de faciliter la mise en réseau des patients.

Mastocytes

Les mastocytes font partie de notre système immunitaire. Ils sont produits par la moelle osseuse et migrent vers les tissus où ils résident à proximité des vaisseaux sanguins et des nerfs. Normalement, on ne trouve pas de mastocytes dans le sang périphérique ; c’est pourquoi la présence de mastocytes ne peut être déterminée qu’au moyen d’une biopsie de tissus (par exemple une biopsie cutanée ou une biopsie de moelle osseuse), et pas par prise de sang. Les mastocytes produisent et stockent des substances (p.ex. de l’histamine) qui peuvent s’en libérer et être à l’origine de divers symptômes.

Mastocytose

Comme expliqué plus haut, les mastocytes font partie du système immunitaire de chaque individu. La différence, avec les patients atteints d’une mastocytose, est que chez ces derniers, les mastocytes se développent et s’accumulent de manière anormale dans un ou plusieurs organes (p.ex. la peau, la moelle osseuse, le foie, la rate). La mastocytose est une maladie qui peut se manifester de différentes manières.

Symptômes

Les symptômes sont souvent causés par la dégranulation ou la libération de substances stockées dans les mastocytes. Les mastocytes contiennent des substances constituées de médiateurs chimiques contenant de l’histamine qui peut être libérée à l’extérieur du mastocyte. Cette libération de médiateurs chimiques peut être à l’origine de symptômes comme des rougeurs, des démangeaisons, des diarrhées, des indigestions, des nausées, des douleurs abdominales, des douleurs musculaires et/ou osseuses, de l’ostéoporose, une modification de la pression artérielle (généralement une chute de pression appelée hypotension), des chocs anaphylactiques, un rythme cardiaque élevé, des maux de tête, des problèmes de concentration, de la fatigue, du « brain fog », des vertiges, de l’irritation, de la colère, de la dépression et de l’anxiété. Pour chaque type de mastocytose, les symptômes peuvent être aigus ou chroniques. La gravité des symptômes peut être minime ou grave et les facteurs déclenchants peuvent fortement varier d’un patient à l’autre. Dans de rares cas de mastocytose systémique agressive et de leucémie à mastocytes, les symptômes sont provoqués par la modification ou la destruction d’organes suite à l’infiltration de mastocytes.

Mastocytose cutanée (CM)

  • Mastocytose cutanée maculo-papulaire (MPCM) (anciennement: Urticaria Pigmentosa (UP)
  • Mastocytose cutanée diffuse (DCM)
  • Mastocytome

Mastocytose systémique (SM)

  • Mastocytose systémique indolente (ISM), (forme la plus commune de mastocytose systémique)

L’Organisation Mondiale de la Santé a subdivisé la mastocytose en deux maladies différentes : la mastocytose cutanée et la mastocytose systémique.

1. Mastocytose cutanée (CM), qui est une maladie exclusivement dermatologique, bien que les symptômes médiateurs puissent être systémiques.

Dans les mastocytoses cutanées, les mastocytes s’accumulent de manière isolée dans la peau. Il s’agit TOUJOURS d’une maladie bénigne. La mastocytose cutanée affecte principalement les enfants ; chez la plupart des enfants, la maladie régresse autour de l’âge de la puberté. Dans la majorité des cas, la mastocytose cutanée n’est pas héréditaire.

a) Différents types de mastocytoses cutanées

Mastocytose cutanée maculo-papulaire (MPCM) (anciennement : Urticaria Pigmentosa (UP). C’est la forme la plus commune. De nombreuses petites taches violettes ou brunes apparaissent sur le corps mais généralement pas sur le visage. Telangiectasia macularis eruptiva perstans (TMEP) est une sous-forme rare de la MPCM. Elle se manifeste sous forme de plaques violettes persistantes sur la peau du corps chez les adultes. Des rougeurs et des démangeaisons accompagnées de fins vaisseaux sanguins peuvent également apparaître.

Mastocytose cutanée diffuse (DCM): une forme très rare présente à la naissance qui se manifeste par une peau épaissie sur laquelle des ampoules contenant du liquide peuvent apparaître.

Mastocytome: rare et seulement dans l’enfance. Il se présente sous la forme d’un nodule simple ou multiple.

b) Critères de diagnostic de la mastocytose cutanée

Le diagnostic de la mastocytose cutanée est basé sur la présence de lésions cutanées cliniques et histologiques et sur l’absence de critères définitifs d’atteinte systémique:

  • exanthème typique, le signe de Darier peut être positif
  • biopsie cutanée positive:
    • infiltration mastocytaire (> 15 mastocytes) ou mastocytes diffus (> 20 mastocytes)
    • ou présence d’une mutation du c-KIT

2. La mastocytose systémique (SM) est une maladie hématologique

La mastocytose systémique est une maladie myéloproliférative. La prolifération mastocytaire est présente dans au moins un organe (normalement dans la moelle osseuse) et doit être confirmée par biopsie. La peau peut être atteinte mais pas nécessairement. La mastocytose systémique apparaît généralement chez les adultes. Elle a une évolution chronique et n’est pas guérissable. Le pronostic de la mastocytose systémique est généralement favorable. Il est rare qu’une forme de mastocytose systémique évolue vers une autre forme. Dans la plupart des cas, la mastocytose systémique n’est pas héréditaire.

a) Différents types de mastocytoses systémiques

La mastocytose systémique indolente (ISM): est la forme la plus courante. Elle peut ne pas évoluer au cours de la vie ni progresser vers une autre forme. Une variante est la mastocytose de la moelle osseuse qui n’est jamais accompagnée d’une atteinte cutanée. Une autre variante est la mastocytose systémique intermédiaire qui se caractérise par une atteinte de mastocytes dans la moelle osseuse plus importante.

La mastocytose systémique associée à une maladie hématologique (SM-AHNMD): différents types de maladies hématologiques peuvent apparaître chez de rares patients. Elles sont diagnostiquées par une analyse de sang de routine ou par une biopsie de moelle osseuse.

La mastocytose systémique agressive (ASM): cette maladie s’accompagne de l’atteinte d’au moins un organe. L’atteinte cutanée est rare.

La leucémie à mastocytes (MCL): il s’agit d’une maladie hématologique extrêmement rare. Il n’y a jamais d’atteinte cutanée.

Il existe deux types de tumeurs graves et différentes, mais très rares. Le sarcome à mastocytes, qui est malin, et le mastocytome extracutané qui est une tumeur bénigne présente ailleurs dans le corps mais pas dans la peau.

b) Critères de diagnostic de la mastocytose systémique

Le diagnostic de la mastocytose systémique est posé sur des patients qui présentent une biopsie d’un organe interne (normalement la moelle osseuse) répondant au moins à un critère majeur et à un critère mineur ou au moins à trois critères mineurs.

Critère majeur

Présence d’infiltrations multifocales denses de mastocytes (> 15 mastocytes).

Critère mineur

  1. plus de 25% des mastocytes présentent une morphologie anormale (forme hélicoïdale ou allongée)
  2. détection d’une mutation du KITsur le codon 816
  3. expression de CD2 et/ou CD25 sur les mastocytes
  4. taux de tryptase sérique > 20ng/ml

c) D’autres examens peuvent inclure: numération-formule sanguine, échographies, densitométrie osseuse (DEXA) et endoscopies.

Traitement

Le traitement de la mastocytose systémique indolente vise à contrôler les symptômes et à éviter les éléments déclenchants à l’origine d’une dégranulation des mastocytes. Le traitement se fait à base d’anti-histaminiques (anti-H1 et anti-H2). Les formes plus agressives de mastocytose systémique sont traitées avec des médicaments cyto-réducteurs spécifiques (chimiothérapie). Rarement, une poly-chimiothérapie, des rayons ou des transplantations de cellules staminales peuvent s’avérer nécessaires.

Einleitung

Der Zweck dieser Webseite ist, Patienten aus unterschiedlichen Ländern eine gemeinsame Plattform anzubieten, um die Möglichkeit zu schaffen, Informationen über Mastozytose zu teilen. Das Ziel ist, eine allgemeine Einführung über diese Erkrankung sowie Links zu unterschiedlichen europäischen Selbsthilfegruppen anzubieten, um ein Netzwerk zu bilden.

Mastzellen

Mastzellen sind ein Teil unseres Immunsystems. Sie stammen aus dem Knochenmark und wandern ins Gewebe, wo sie in der Nähe von Blutgefäßen und Nerven vorkommen. Normalerweise kommen Mastzellen nicht im Blut vor; deshalb können sie nur mit Hilfe von Biopsien nachgewiesen werden (z.B. in der Haut oder im Knochenmark), nicht aber in einer Blutuntersuchung. Mastzellen produzieren und speichern Stoffe (z.B. Histamin), die ausgeschüttet werden und zu Symptomen führen können.

Mastozytose

Wie bereits erwähnt sind Mastzellen Teil des Immunsystems und kommen demnach bei jedem Menschen vor. Der Unterschied zwischen gesunden Menschen und Mastozytose Patienten besteht darin, dass letztere eine unnormale Vermehrung und Ansammlung von Mastzellen in einem oder mehreren Organsystemen (z.B.: Haut, Knochenmark, Leber, Milz) haben. Mastozytose ist eine Erkrankung, die sich sehr unterschiedlich präsentieren kann.

Symptome

Die Symptome werden durch das Ausschütten der Stoffe, die sich in den Mastzellen befinden, verursacht. Mastzellen enthalten chemische Stoffe (z.B. Histamin), die sie ausschütten können. Eine solche Ausschüttung kann u.a. zu folgenden Symptomen führen: Flush, Juckreiz, Durchfall, Übelkeit, Bauchschmerzen, Muskel- und Knochenschmerzen, Osteoporose, Blutdruckschwankungen (in der Regel Blutdruckabfall), Anaphylaxie, hohe Herzfrequenzen, Kopfschmerzen, Konzentrationsprobleme, Fatigue, Schwindel, Reizbarkeit, Wut, Depression und Angst. Bei jeder Form der Mastozytose können die Symptome akut oder chronisch vorkommen. Die Schwere der Symptome kann mild oder lebensbedrohlich sein. Symptome können von Triggern ausgelöst werden, die von Patient zu Patient unterschiedlich sind. Im seltenen Fall der aggressiven systemischen Mastozytose und bei der Mastzellleukämie kommen die Symptome auch aufgrund der hohen Mastzellinfiltration der Organe vor.

Hautmastozytose (CM)

  • Maculopalulare kutane Mastozytose (MPCM) (früher: Urtikaria pigmentosa (UP))
  • Diffuse kutane Mastozytose (CDM)
  • Mastozytom

Systemische Mastozytose (SM)

  • Indolente systemische Mastozytose (ISM), (die häufigste Form der systemischen Mastozytose)
    • Isolierte Knochenmarksmastozytose (BMM)
    • Smouldering systemische Mastozytose (SSM)
  • Systemische Mastozytose mit einer begleitenden klonalen nicht-mastzellartigennBluterkrankung (SM-AHNMD)

Die WHO-Klassifikation teilt die Mastozytose in zwei unterschiedliche Erkrankungen ein: I. Hautmastozytose oder II. systemische Mastozytose.

1. Hautmastozytose (CM)– ist ausschließlich eine dermatologische Erkrankung– Die Symptome können jedoch auch systemisch sein.

Bei der Hautmastozytose kommt die Mastzellvermehrung nur in der Haut vor.

Hautmastozytosen sind praktisch immer gutartige Erkrankungen.

Hautmastozytosen kommen in erster Linie bei Kindern vor und bei vielen heilen sie rund um die Pubertät aus. In den allermeisten Fällen ist die Hautmastozytose nicht vererbbar.

a) Verschiedene Formen der kutanen Mastozytose

Die Maculopapulare kutane Mastozytose (MPCM) (früher: Urtikaria pigmentosa (UP)) ist die häufigste Form der Hautmastozytose. Hier kommen kleine rosafarbene oder bräunliche Flecken im Bereich des Körpers nicht aber des Gesichts vor.

Die Telangiectasia macularis eruptiva perstans (TMEP) ist eine seltene Unterform der MPCM. Sie wird bei Erwachsenen gefunden und zeigt sich in kleinen rosafarbenen Hautflecken, die von kleinen Blutgefäßen durchzogen werden.

Diffuse kutane Mastozytose (DCM) ist eine sehr seltene Form der Hautmastozytose, aufgrund der hohen Mastzelllast in der Haut ist diese verdickt. Gerne kommt es zur Blasenbildung.

Mastozytom

Ein Mastozytom ist selten und kommt zumeist in der Kindheit vor. Es erscheint als knotiger oder plattenartiger Herd. Ein Kind kann ein oder mehrere Mastozytome entwickeln.

b) Diagnose Kriterien der kutanen Mastozytose

Die Diagnose der Hautmastozytose basiert auf klinischen und histologischen Hauterscheinungen sowie auf der Abwesenheit von definitiven Kriterien der systemischen Mastozytose.

  • typisches Hautexanthem, das Darier Zeichen kann positiv sein
  • positive Hautbiopsie:
    • Mastzellherde (>15 Mastzellen) oder verstreut liegende Mastzellen (>20 Mastzellen)
    • oder das Auffinden der KITMutation

2. Systemische Mastozytose (SM)– ist eine hämatologische Erkrankung

Die systemische Mastozytose ist eine myeloproliferative Neoplasie. Die Mastzellvermehrung kommt in mindestens einem inneren Organ (i.d.R. Knochenmark) vor und wird dort durch eine Biopsie festgestellt. Hinzu kann eine Beteiligung der Haut kommen, dies muss aber nicht zwingend der Fall sein. Die systemische Mastozytose kommt in der Regel bei Erwachsenen vor, sie verläuft chronisch, ist also nicht heilbar. Die Prognose der systemischen Mastozytose ist in der Regel gut. Selten geht eine Form der systemischen Mastozytose in eine andere über. In den meisten Fällen ist die systemische Mastozytose nicht vererbbar.

a) Verschiedene Formen der systemischen Mastozytose

Die indolente systemische Mastozytose (ISM) ist die häufigste Form. Selten geht sie in eine andere Form über. Eine Unterform bildet die isolierte Knochenmarks Mastozytose (BMM), die nie von einer Hautbeteiligung begleitet wird.

Eine weitere Unterform ist die smouldering systemische Mastozytose (SSM), sie geht mit einer höheren Knochenmarksbeteiligung einher.

Systemische Mastozytose mit einer begleitenden nichtmastzell-artigen Bluterkrankung (SM-AHNMD): hier wird die systemische Mastozytose von einer weiteren Knochenmarkserkrankung begleitet. Die Diagnose wird mittels einer Knochenmarksbiopsie oder einer Routineblut Untersuchung gestellt.

Bei der aggressiven systemischen Mastozytose (ASM) ist mindestens ein Organ in seiner Funktion eingeschränkt. Eine Hautbeteiligung kommt selten vor.

Die Mastzellleukämie (MCL) ist eine sehr sehr seltene hämatologische Erkrankung. Sie kommt niemals mit einer Hautbeteiligung vor.

Es gibt zwei unterschiedliche und sehr seltene solide Mastzelltumore. Zum einen handelt es sich um das so genannte Mastzellsarkom, welches bösartig ist – zum anderen um das extrakutane Mastozytom, welches ein gutartiger Tumor darstellt, der irgendwo im Körper und nicht in der Haut gefunden wird.

b) Die Diagnose Kriterien der systemischen Mastozytose

sind dann erfüllt, wenn das Hauptkriterium und eines der Nebenkriterien in der Biopsie eines inneren Organes (zumeist Knochenmark) gefunden werden; oder wenn zumindest drei Nebenkriterien erfüllt sind.

Hauptkriterium

Das Vorhandensein von dicht aneinander gelagerten Mastzellherden (> 15 Mastzellen).

Nebenkriterien

  1. Mehr als 25% der Mastzellen haben eine atypische Form: spindel- oder ovalförmig
  2. Auffinden der KITMutation im Codon 816
  3. CD2 und/oder CD25 werden auf den Mastzellen gefunden.
  4. Serum Tryptase > 20 ng/ml.

c) Weitere Untersuchungen können sein: Differentialblutbild, Ultraschall, Knochendichtemessung (z.B. mittels Dexa) und Endoskopien

Therapie

Die Therapie der indolenten systemischen Mastozytose erfolgt symptomorientiert und besteht zunächst im Vermeiden von individuellen Triggern, die eine Mastzellausschüttung verursachen können. In zweiter Linie besteht sie aus der Einnahme von Histamin-Rezeptor-Antagonisten.

Aggressivere Formen werden mit spezifischen zytoreduktiven oder zielgerichteten Medikamenten behandelt. Selten kann auch eine Polychemotherapie, Strahlentherapie, oder Stammzelltransplantation notwendig werden.

Εισαγωγή

Σκοπός αυτής της ιστοσελίδας είναι να προσφέρει στους ασθενείς από διαφορετικές χώρες ένα κοινό πλαίσιο για την αναζήτηση και ανταλλαγή πληροφοριών σχετικά με τη μαστοκυττάρωση.
Στόχος της είναι να παρέχει μια γενική εισαγωγή για τη νόσο, καθώς και συνδέσεις σε διαφορετικές Ευρωπαϊκές ιστοσελίδες υποστήριξης ασθενών για την προώθηση της ανθρώπινης επικοινωνίας μέσω διαδικτύου.

Μαστοκύτταρα

Τα μαστοκύτταρα αποτελούν μέρος του ανοσοποιητικού μας συστήματος. Παράγονται στο μυελό των οστών και μεταναστεύουν στους ιστούς, όπου εγκαθίστανται κοντά στα αιμοφόρα αγγεία ή νεύρα. Συνήθως τα μαστοκύτταρα δεν βρίσκονται στο περιφερικό αίμα. Συνεπώς, η παρουσία των μαστοκυττάρων καθορίζεται αποκλειστικά και μόνο με βιοψία ιστού (π.χ. δέρματος ή μυελού των οστών) και όχι με δείγμα αίματος. Τα μαστοκύτταρα έχουν τη δυνατότητα να παράγουν και να αποθηκεύουν ουσίες (π.χ. ισταμίνη), οι οποίες μπορούν να αποδεσμευτούν και να προκαλέσουν συμπτώματα.

Μαστοκυττάρωση

Όπως προαναφέρθηκε τα μαστοκύτταρα αποτελούν μέρος του ανοσοποιητικού συστήματος κάθε ανθρώπου. Η διαφορά στους ασθενείς με μαστοκυττάρωση είναι ότι παρουσιάζουν μια μη φυσιολογική ανάπτυξη και συσσώρευση των μαστοκυττάρων σε ένα ή περισσότερα οργανικά συστήματα (π.χ. δέρμα, μυελό των οστών, ήπαρ, σπλήνα). Η μαστοκυττάρωση είναι μια διαταραχή που παρουσιάζεται με πολλούς διαφορετικούς τρόπους

Συμπτώματα

Τα συμπτώματα συχνά οφείλονται στην αποκοκκίωση ή στην απελευθέρωση των ουσιών (σε κοκκία) που αποθηκεύονται στα μαστοκύτταρα. Τα μαστοκύτταρα περιέχουν κοκκία με χημικές ουσίες, συμπεριλαμβανομένου της ισταμίνης, η οποία μπορεί να απελευθερωθεί εκτός των μαστοκυττάρων. Αυτή η απελευθέρωση χημικών ουσιών μπορεί να οδηγήσει σε συμπτώματα όπως ερύθημα, κνησμό, διαρροϊκές κενώσεις, δυσπεψία, ναυτία, κοιλιακό άλγος, μυϊκό άλγος, οστικό άλγος, οστεοπόρωση, μεταβολές της αρτηριακής πίεσης του αίματος (συνήθως πτώση γνωστή ως υπόταση), αναφυλαξία, ταχυκαρδία, πονοκεφάλους, δυσκολία συγκέντρωσης, κόπωση, ομίχλη εγκεφάλου, ζάλη, ευερεθιστικότητα, θυμό, κατάθλιψη και φόβο. Σε κάθε τύπο μαστοκυττάρωσης τα συμπτώματα μπορεί να είναι οξέα ή χρόνια. Η σοβαρότητα των συμπτωμάτων μπορεί να είναι ήπια ή απειλητική για τη ζωή και μπορεί να οφείλεται σε εκλυτικούς παράγοντες που διαφέρουν σημαντικά από ασθενή σε ασθενή. Στη σπάνια περίπτωση της συστηματικής επιθετικής μαστοκυττάρωσης και στη λευχαιμία εκ μαστοκυττάρων, τα συμπτώματα προκύπτουν λόγω των αλλαγών και της καταστροφής των οργάνων μέσω της διήθησης των μαστοκυττάρων.

Ο Παγκόσμιος Οργανισμός Υγείας έχει κατηγοριοποιήσει τη Μαστοκυττάρωση σε δύο διαφορετικές παθήσεις: τη Δερματική Μαστοκυττάρωση και τη Συστηματική Μαστοκυττάρωση.

I. Δερματική Μαστοκυττάρωση(CM), η οποία είναι αποκλειστικά δερματολογική πάθηση, μολονότι τα συμπτώματα από την απελευθέρωση των μεσολαβητών μπορεί να είναι συστηματικά.

Στη δερματική μαστοκυττάρωση η συσσώρευση των μαστοκυττάρων περιορίζεται στο δέρμα. Πρόκειται σε ΟΛΕΣ ΤΙΣ ΠΕΡΙΠΤΩΣΕΙΣ για καλοήθη ασθένεια. Η δερματική μαστοκυττάρωση εμφανίζεται κυρίως στα παιδιά και στα περισσότερα παιδιά υποχωρεί στην εφηβεία. Στις περισσότερες περιπτώσεις η δερματική μαστοκυττάρωση δεν είναι κληρονομική.

α) Διαφορετικοί τύποι της δερματικής μαστοκυττάρωσης

Κηλιδοβλατιδώδης δερματική μαστοκυττάρωση (MPCM) (πρώην Urticaria Pigmentosa (UP)): αποτελεί τη συνηθέστερη μορφή. Αρκετές ροζ ή καφέχροες κηλίδες ή βλατίδες εμφανίζονται στο σώμα και όχι συνήθως στο πρόσωπο Εξανθηματική κηλιδώδης τελαγγειεκτασία (TMEP): πρόκειται για μια σπάνια υποκατηγορία MPCM. Εμφανίζεται στους ενήλικες ως μόνιμες ροζ κηλίδες του δέρματος στο σώμα με συνοδό ερύθημα, κνησμό και τελαγγειεκτασίες.

Διάχυτη δερματική μαστοκυττάρωση (DCM): μια πολύ σπάνια μορφή, παρούσα κατά τη γέννηση, με πάχυνση του δέρματος και εμφάνιση φυσαλίδων.

Μαστοκυττάρωμα: σπάνιο εμφανίζεται στην παιδική ηλικία, μονήρες ή πολλαπλό επηρμένο οζίδιο.

β) Διαγνωστικά κριτήρια της δερματικής μαστοκυττάρωσης

Η διάγνωση της δερματικής μαστοκυττάρωσης βασίζεται στη κλινική και ιστολογική εξέταση των δερματικών βλαβών καθώς και στην απουσία οριστικών κριτηρίων συστηματικής συμμετοχής:

  • Τυπικό εξάνθημα, το σημείο Darier μπορεί να είναι θετικό
  • Θετική βιοψία δέρματος:
    • μαστοκυτταρική διήθηση (> 15 μαστοκύτταρα) ή διάσπαρτα μαστοκύτταρα (> 20 μαστοκύτταρα ανά ιστό)
    • ή ανίχνευση μετάλλαξης KIT

2. Η συστηματική Μαστοκυττάρωση (SM) είναι αιματολογική νόσος

Η συστηματική μαστοκυττάρωση είναι ένα μυελοϋπερπλαστικό νόσημα. Ο πολλαπλασιασμός των μαστοκυττάρων βρίσκεται σε ένα τουλάχιστον όργανο (συνήθως στο μυελό των οστών) και θα πρέπει να επιβεβαιωθεί με βιοψία. Δερματική συμμετοχή μπορεί να συνυπάρχει αλλά δεν είναι απαραίτητα παρούσα.
Η συστηματική μαστοκυττάρωση είναι συνήθως νόσος των ενηλίκων. Έχει μια χρόνια πορεία και δεν είναι θεραπεύσιμη. Η πρόγνωση της συστηματικής μαστοκυττάρωσης είναι συνήθως ευνοϊκή. Σπάνια μια μορφή συστηματικής μαστοκυττάρωσης εξελίσσεται σε μια άλλη μορφή. Στις περισσότερες περιπτώσεις, η συστηματική μαστοκυττάρωση δεν είναι κληρονομική.

α) Διαφορετικοί τύποι της Συστηματικής Μαστοκυττάρωσης

Ανώδυνη συστηματική μαστοκυττάρωση (ISM):η συνηθέστερη μορφή. Μπορεί να παραμείνει αμετάβλητη δια βίου και σπάνια εξελίσσεται σε άλλον τύπο. Υποκατηγορία αποτελεί η Μαστοκυττάρωση Μυελού των Οστών, η οποία ποτέ δεν συνοδεύεται από τη συμμετοχή του δέρματος. Άλλη υποκατηγορία είναι η Λανθάνουσα Συστηματική Μαστοκυττάρωση, η οποία υποδεικνύει ένα υψηλότερο μαστοκυτταρικό φορτίο.

Συστηματική Μαστοκυττάρωση με συνοδό αιματολογικό νόσημα κλωνικού τύπου μη μαστοκυτταρικής σειράς (SM-AHNMD): διαφορετικοί τύποι αιματολογικών διαταραχών μπορούν να αναπτυχθούν σε κάποιους ασθενείς. Ανευρίσκονται σε μια συνηθισμένη εξέταση αίματος ή με βιοψία μυελού των οστών.

Επιθετική Συστηματική Μαστοκυττάρωση (ASM): η ασθένεια αυτή συνοδεύεται από ανεπάρκεια τουλάχιστον ενός οργάνου. Η συμμετοχή του δέρματος είναι σπάνια.

Λευχαιμία εκ μαστοκυττάρων (MCL): πρόκειται για μια εξαιρετικά σπάνια αιματολογική νόσο. Δερματικές βλάβες δεν συνυπάρχουν ποτέ.

Υπάρχουν δύο διαφορετικοί και πολύ σπάνια συμπαγείς μαστοκυτταρικοί όγκοι: Σάρκωμα εκ μαστοκυττάρων, κακοήθης όγκος και το Εξωδερματικό Μαστοκυττάρωμα, καλοήθης όγκος που βρίσκεται σε άλλα σημεία του σώματος εκτός του δέρματος.

β) Διαγνωστικά κριτήρια της Συστηματικής Μαστοκυττάρωσης

Η διάγνωση της συστηματικής μαστοκυττάρωσης τίθεται σε ασθενείς των οποίων η βιοψία ενός εσωτερικού οργάνου (συνήθως μυελού των οστών) πληροί τουλάχιστον ένα μείζον και ένα έλασσον κριτήριο ή τουλάχιστον τρία ελάσσονα κριτήρια.

Μείζον κριτήριο

Πολυεστιακή πυκνή μαστοκυτταρική διήθηση (> 15 μαστοκύτταρα ανά άθροιση)

Ελάσσονα κριτήρια

  1. περισσότερο από το 25% των μαστοκυττάρων δείχνουν μια άτυπη μορφολογία (ατρακτοειδή ή επιμήκη)
  2. ανίχνευση KITμετάλλαξης στο κωδικόνιο 816
  3. μαστοκύτταρα που εκφράζουν CD2 και / ή CD25
  4. τρυπτάση ορού> 20ng/ml

γ) Περαιτέρω δοκιμασίες συμπεριλαμβάνουν: γενική αίματος, υπερηχογραφική μελέτη, μέτρηση οστικής πυκνότητας (DEXA scan) και ενδοσκοπήσεις.

Θεραπεία

Η θεραπεία της συστηματικής μαστοκυττάρωσης προσανατολίζεται στην αντιμετώπιση των συμπτωμάτων και συνίσταται τόσο στην αποφυγή μεμονωμένων εκλυτικών παραγόντων που μπορούν να προκαλέσουν αποκοκκίωση των μαστοκυττάρων, όσο και στη φαρμακευτική αγωγή με ανταγωνιστές των υποδοχέων της ισταμίνης. Οι πιο επιθετικές μορφές συστηματικής μαστοκυττάρωσης αντιμετωπίζονται με ειδικά κυτταρομειωτικά φάρμακα (χημειοθεραπεία). Σπάνια χρήζουν πολυχημειοθεραπείας, ακτινοβολίας ή μεταμόσχευσης βλαστικών κυττάρων.

Hízósejtek

A hízósejtek az immunrendszer részei. A csontvelőben termelődnek és a szövetekbe kerülnek, ahol az erek vagy az idegek közelében tartózkodnak. Normál esetben hízósejtek nem találhatók a perifériás vérben; ezért a hízósejtek jelenléte kizárólag szöveti biopszia (pl: bőr vagy csontvelő) révén határozható meg és nem vérmintából. A hízósejtek különböző anyagokat termelnek és tárolnak (pl: hisztamint), melyek onnan felszabadulhatnak és ennek nyomán tünetek jelentkezhetnek.

Masztocitózis

Tudomásunk szerint a hízósejtek része minden egyes ember immun rendszerének. A mastocitózisos betegek esetében annyi a külöbség , hogy a hízósejtek abnormálisan növekednek és felhalmozódnak egy vagy több szervben (pl: bőr, csontvelő, máj, lép). A Mastocytosis egy olyan betegség aminek különféle megnyilvánulási formája ismert.

Tünetek

A tüneteket gyakran a degranuláció az a hízósejtekben tárolt anyagok kiáramlása okozza. A hízósejtek egy sor kémiai anyagot tartalmaznak, beleérte a hisztamint is, ami a hízósejtből a sejten kívülre kerül. Ennek az anyagnak a hízósejten kívülre kerülése olyan tünetekhez vezethet, mint a kipirulás, viszketés, hasmenés, emésztési zavar, hányinger, hasi fájdalom, izom / csont-fájdalom, a csontritkulás, a vérnyomás változása (csökkenése általában alacsony vérnyomás ismert), anafilaxia, a magas szívfrekvencia, fejfájás, koncentrációs problémák, fáradtság, agyi köd, szédülés , ingerlékenység, düh, depresszió és a félelem. A Nemzetközi Világszervezet két különböző betegségként sorolta be a Masztocitozis: Bőr Masztocitozis és Szisztémás Masztocitozis.

1. Bőr Mastocytosis (CM), ami kizárólag bőrbetegség, bár a kiszabaduló mediátorok okozta tünetek szisztémás hatásúak lehetnek A bőr mastocytosis esetén a hízósejtek felhalmozódása csak a bőrben zajlik. 2. Szisztémás Mastocytosis (SM) egy hematológiai betegség

A szisztémás mastocytosis egy mieloproliferatív daganat. Hízósejt proliferáció megtalálható legalább egy szervben (általában a csontvelőben), és meg kell erősíteni biopsziával. Bőr involváció is lehetséges – de nem szükséges, hogy jelen legyen. A szisztémás mastocytosis általában felnőtteknél található meg, Krónikus lefolyású, és nem gyógyítható. A szisztémás mastocytosis prognózisa általában kedvező. Ritkán a szisztémás masztocitózis előrehaladtával az egyik formája átfejlődhet a másikba. a) Különböző típusú szisztémás masztocitózis

Két különböző és nagyon ritka hízósejt tumor létezik. Hízósejt Szarkóma, ami rosszindulatú és Extracutan Mastocytoma, amely egy jóindulatú daganat,a testen belül bárhol megtalálható, de a bőrön nem.

b) Szisztémás Mastocytosis diagnosztikai kritériumai

A szisztémás mastocytosis deiagnózisa azokon a betegeken alapul akiknek egy belső szervi biopsziája (általában csontvelő) megfelel legalább egy nagy és egy kis, illetve legalább három kis kritériumnak.

Nagy kritérium

Multifokális sűrű beszűrődése a hízósejteknek (> 15 hízósejt csoportonként)

Kis kritériumok

  1. a hízósejtek több, mint 25%-a rendellenes morfológiát mutat (orsó alakú vagy hosszúkás)
  2. egy KIT mutáció észlelése 816 kodonnál
  3. hízósejtek CD2-és / vagy CD25-öt mutatnak
  4. szérum triptáz> 20ng/ml

További tesztek lehetnek: teljes vérkép, ultrahang, DEXA vizsgálat és endoszkópia.

c) További vizsgálatok: teljes vérkép, ultrahang, osteodenzitometria, endoscopia

Kezelés

Az indolens szisztémás Mastocytosis kezelése tünet orientált, és azon egyéni kiváltó okok elkerüléséből áll, amelyek hízósejt degranulációt okozhatnak. Kezelés hisztamin receptor antagonistákkal. Agresszívebb formáját a szisztémás mastocytosis-nak speciális citoreduktív gyógyszerekkel (kemoterápia) kezelik. Ritkán polikemoterápia, sugárzás vagy őssejt-transzplantáció lehet szükséges.

I mastociti

I mastociti sono parte del nostro sistema immunitario. Sono prodotti nel midollo osseo e vanno nei tessuti dove si situano vicino ai vasi sanguigni o ai nervi. Normalmente i mastociti non si trovano nel sangue periferico; perciò la presenza di mastociti è determinata esclusivamente attraverso una biopsia dei tessuti (ad esempio la pelle o del midollo osseo) e non attraverso un prelievo sanguigno. I mastociti producono e accumulano sostanze (come l’istamina), che possono essere rilasciate e possono manifestarsi con dei sintomi.

Mastocitosi

Come spiegato, i mastociti fanno parte del sistema immunitario di ogni individuo. La differenza nei malati di mastocitosi è che questi hanno un’abnorme proliferazione e accumulo di mastociti in uno o più organi e tessuti (pelle, midollo, fegato, milza). La mastocitosi è un disturbo che si può manifestare in diversi modi.

Sintomi

I sintomi sono spesso causati dalla degranulazione che consiste nel rilascio di sostanze accumulate nei granuli dei mastociti. I mastociti contengono depositi (o granuli) di mediatori chimici, tra cui l’istamina, che possono essere rilasciati all’esterno del mastocita. Questo rilascio può portare a sintomi quali rossore, prurito, diarrea, vomito, nausea, dolori addominali, dolori muscolari e ossei, osteoporosi, sbalzi di pressione sanguigna (di solito un notevole abbassamento detto ipotensione), anafilassi, tachicardia, emicrania, problemi nella concentrazione, affaticamento, annebbiamento mentale, vertigini, irritazione, rabbia, depressione e paura. In ogni tipo di mastocitosi i sintomi possono essere acuti o cronici. L’entità dei sintomi può essere minima o particolarmente severa e può variare molto da paziente a paziente. Nei rari casi di mastocitosi sistemica acuta e nella leucemia mastocitaria, i sintomi sono particolarmente gravi e sono dovuti soprattutto all’invasione del midollo osseo, del fegato e della milza da parte dei mastociti.

L’Organizzazione mondiale della Sanità ha suddiviso la mastocitosi in due diverse malattie: la mastocitosi cutanea e la mastocitosi sistemica.

1. La mastocitosi cutanea, che è una malattia esclusivamente dermatologica, sebbene i sintomi legati ai mediatori possano essere generali. Nella mastocitosi cutanea l’accumulo di mastociti è limitato alla pelle. E’ quasi sempre una malattia benigna. La mastocitosi cutanea si rileva soprattutto nei bambini e nella maggior parte dei casi regredisce nel periodo della pubertà. Nella maggior parte dei casi la mastocitosi cutanea non è ereditaria.

a) Diversi tipi di mastocitosi cutanea

la Mastocitosi cutanea maculopapulare (MPCM) (anche definita Orticaria Pigmentosa (UP)) è il tipo più comune. Si manifestano sul corpo, risparmiando di solito il viso, delle piccole macchie o rigonfiamenti rosa o marroni. La Telangiectasia macularis eruptiva perstans (TMEP) è una rara sottoforma di MPCM. Si manifesta in adulti con persistenti chiazze rosa sul corpo. Si possono notare rossore e prurito con formazione di sottili vasi sanguigni tipo capillari dilatati.

la Mastocitosi cutanea diffusa (DCM): una forma molto rara presente alla nascita in cui la cute è inspessita con tendenza a formare vesciche a contenuto liquido.

Mastocitoma: raro, si manifesta generalmente in età infantile. Può presentarsi come un nodulo in rilievo singolo o multiplo.

b) Criteri diagnostici della mastocitosi cutanea

La diagnosi di Mastocitosi cutanea si fonda sul riscontro delle tipiche lesioni cutanee e sulla mancanza di segni di interesse sistemico:

  • esantema tipico, il segno di Darier (comparsa di gonfiore e rossore dopo sfregamento sulla macchiolina) è quasi sempre positivo
  • biopsia cutanea positiva:
  • infiltrazione di mastociti aggregati (> 15 mastociti/aggregato) o sparsi (>20 mastociti/campo microscopico)
  • rilevamento di una mutazione del gene KITsulla biopsia cutanea

La mastocitosi sistemica (SM) è una malattia ematica

La mastocitosi sistemica è una malattia mieloproliferativa. La proliferazione dei mastociti è presente in almeno un organo (normalmente nel midollo osseo) e deve essere confermata dalla biopsia. L’interessamento cutaneo può essere presente, ma non è necessario per la diagnosi. La mastocitosi sistemica si riscontra normalmente negli adulti. Ha un decorso cronico e non è curabile. Tuttavia, la prognosi della mastocitosi sistemica è di solito benigna. Raramente una forma di mastocitosi sistemica indolente evolve in una aggressiva. Nella maggior parte dei casi la mastocitosi sistemica non è ereditaria.

a) Diversi tipi di mastocitosi sistemica

La mastocitosi sistemica indolente (ISM): il tipo più comune. Può rimanere immutata per tutta la vita e raramente progredisce in un altro tipo. Una sottovariante è la mastocitosi del midollo osseo, che non è mai accompagnata da un interessamento cutaneo. Un’altra variante è la mastocitosi sistemica intermedia (smoldering), che mostra un numero particolarmente elevato di mastociti nel midollo od in altri organi.

La mastocitosi sistemica associata a un disturbo ematico (SM-AHNMD): in alcuni pazienti la mastocitosi si associa ad altre patologie ematologiche come linfomi, mielodisplasie od altre sindromi mieloproliferative. Queste patologie vengono diagnosticate con la biopsia del midollo osseo.

La mastocitosi sistemica aggressiva (ASM): questa malattia è caratterizzata dal progressivo danneggiamento e dalla disfunzione del midollo osseo, del fegato, della milza o dalla comparsa di lesioni ossee (osteolisi). L’interessamento della cute è raro.

La leucemia mastocitaria (MCL): questa è una malattia ematica assai rara. Le lesioni cutanee non sono mai presenti.

Ci sono due diversi tumori molto rari di tipo mastocitario. Il sarcoma mastocitario, che è maligno, e il mastocitoma extracutaneo, che è un tumore meno aggressivo, che può localizzarsi in varie parti del corpo ma non sulla cute.

b) Criteri diagnostici della mastocitosi sistemica

La diagnosi di mastocitosi sistemica si conferma in pazienti la cui biopsia di un organo interno (di norma il midollo osseo) soddisfa il criterio principale ed almeno uno secondario oppure almeno tre criteri secondari

Criterio principale

Presenza di infiltrati multifocali di mastociti (> 15 mastociti per aggregato)

Criteri secondari

  1. più del 25% dei mastociti nel midollo con morfologia anormale (affusolata o allungata)
  2. rilevamento di una mutazione KITal codone 816
  3. espressione di marcatori aberranti (CD2 e/o CD25) sui mastociti
  4. valori di triptasi sierica > 20ng/ml

c) Ulteriori esami possono comprendere: esami ematochimici completi (emocromo, funzionalità epatica, coagulazione), l’ecografia addominale, la mineralometria ossea (MOC) con tecnica DEXA ed endoscopie (gastrica e del colon).

Trattamento

La terapia della mastocitosi sistemica indolente è mirata a controllare la sintomatologia e consiste nell’evitare cause scatenanti che possano attivare la degranulazione dei mastociti. I farmaci più usati sono gli antistaminici (anti-H1 ed anti-H2). In qualche caso possono essere necessarie basse dosi di cortisone. Le forme più aggressive di mastocitosi sistemica sono trattate con specifici trattamenti citoriduttivi (chemioterapia). In rarissimi casi rari casi possono essere necessari polichemioterapia, radioterapia o il trapianto di cellule staminali.

Innledning

Mastceller

Mastceller er en del av vårt immunsystem. De dannes i benmargen, og transporteres til forskjellige vev, der de finnes nær blodårer og nerver. Mastocytose

Som nevnt er mastceller en del av hvert enkelt menneskes immunsystem. Mastocytose er en sykdom som kan vise seg på mange ulike måter.

Symptomer

Symptomene skyldes ofte frisetting av stoffer (mediatorer) lagret i mastcellene. Mastceller inneholder små pakker (‘blærer’ eller granula) med disse kjemiske stoffene, bl.a. histamin, som kan slippes ut utenfor cellen. Denne kjemiske frisettingen kan gi symptomer som rødme, kløe, diaré, kvalme, magesmerter, muskel-/bensmerter, benskjørhet, forandringer i blodtrykk (vanligvis blodtrykksfall), allergisjokk, rask puls, hodepine, konsentrasjonsvansker, sløvhet, svimmelhet,, irritabilitet, sinne, depresjon og angst. I alle former av mastocytose kan symptomene være akutte eller kroniske. Graden av symptomer kan være lette eller livstruende og kan skyldes utløsende faktorer som er forskjellige fra pasient til pasient. I de sjeldne tilfellene av aggressiv systemisk mastocytose og mastcelle leukemi skyldes symptomene forandring og ødeleggelse i organer på grunn av innvekst av mastceller.

WHO har klassifisert mastocytose i to sykdomsgrupper: Kutan mastocytose og systemisk mastocytose.

1. Kutan mastocytose (CM)er en ren hudsykdom, selv om symptomene kan komme fra hele kroppen på grunn av frisetting av mediatorer i blodbanen. Ved kutan mastocytose er det bare mastcelleopphopning i huden. Det er ALLTID en godartet sykdom. Kutan mastocytose finnes hovedsakelig hos barn, og hos de fleste vil sykdommen gå helt tilbake rundt puberteten. Tilstanden er vanligvis ikke arvelig.

2. Systemisk mastocytose (SM) er en blodsykdom

Systemisk mastocytose er en myeloproliferativ sykdom. Mastcellevekst finnes i minst ett organ (vanligvis benmarg) og må bekreftes med vevsprøve. Hud kan være affisert, men det er ikke en betingelse. Systemisk mastocytose sees vanligvis hos voksne. Den har et kronisk forløp og kan ikke helbredes. Prognosen er vanligvis god. a) Forskjellige former for systemisk mastocytose

b) Diagnostiske kriterier for systemisk mastocytose

Diagnosen systemisk mastocytose baseres på vevsprøve av et indre organ (vanligvis benmarg) der minst ett hovedkriterium og ett underkriterium oppfylles eller der minst tre underkriterier oppfylles.

Hovedkriterier

  • Grupper av tette infiltrater av mastceller (>15 mastceller per cellesamling)

Underkriterier

  • Mer enn 25% av mastcellene viser abnorm morfologi (spoleformete eller langstrakte)
  • Påvisning av en KITmutasjon ved kodon 816
  • Mastcellene viser CD 2 og/eller CD 25
  • Serum tryptase > 20ng/ml

c) Ytterligere undersøkelser kan være: Full celletelling i blod, ultralydundersøkelse, DEXA scan (bentetthetsmåling) og skopiundersøkelser.

Behandling

Behandling av indolent systemisk mastocytose er rettet mot symptomer og består i å unngå utløsende faktorer som kan gi frisetting av mastcellemediatorer. Aktuelle medisiner er antihistaminer (allergimedisiner). Mer aggressive former for systemisk mastocytose kan behandles med cellegift. Komórki tuczne (mastocyty)

Komórki tuczne należą do naszego układu odpornościowego. Są one wytwarzane w szpiku kostnym i przemieszczają się do tkanek, gdzie przebywają blisko naczyń krwionośnych lub nerwów. Zwykle mastocyty nie występują we krwi obwodowej. Z tego względu obecność mastocytów wykrywa się wyłącznie podczas biopsji tkanki (np. skóry lub szpiku kostnego), a nie podczas badania krwi. Mastocyty wytwarzają i gromadzą substancje (tj. histamina), które mogą być uwalniane i mogą wywoływać objawy choroby.

Mastocytoza

Jak wyjaśniono powyżej, mastocyty są częścią układu odpornościowego każdego człowieka. W przypadku pacjentów chorych na mastocytozę dochodzi do nadmiernego rozrostu i gromadzenia się mastocytów w jednym lub kilku organach (np. skórze, szpiku kostnym, wątrobie, śledzionie). Objawy

Objawy mogą wynikać z nacieczenia lub uwolnienia substancji (ziaren) zgromadzonych w mastocytach. Mastocyty składają się z ziarnistości (lub ziaren) zawierających substancje chemiczne, w tym histaminę, które mogą zostać uwolnione poza mastocyty. W przypadku mastocytozy skórnej zgromadzone mastocyty są odizolowane w skórze. Mastocytoza skórna jest ZAWSZE chorobą łagodną. Występuje ona głównie u dzieci i często ulega remisji w okresie dojrzewania. W większości przypadków mastocytoza skórna nie jest chorobą dziedziczną.

a) Różne typy mastocytozy skórnej

Pokrzywka barwnikowa (MPCM) (wcześniej nazywana: Urticaria Pigmentosa(UP)) jest najczęściej spotykaną formą choroby. Liczne, małe, różowe lub brązowe plamy lub guzy występują zwykle na ciele, rzadko na twarzy. Rzadką postacią pokrzywki barwnikowej jest teleangiektazja plamista wysypkowa trwała (TMEP), której objawów zalicza się trwałe różowe plamy na ciele u dorosłych pacjentów. Do objawów można też zaliczyć rumień i świąd oraz obecność rozszerzonych naczyń krwionośnych.

Rozlana mastocytoza skórna (DCM): bardzo rzadka postać choroby, obecna najczęściej już przy urodzeniu – objawia się zgrubieniami skóry na której często pojawiają się pęcherze.

Mastocytoma: rzadka postać choroby występująca u dzieci. Występuje w postaci pojedynczych lub licznych guzków.

b) Kryteria diagnostyczne mastocytozy skórnej

Diagnoza mastocytozy skórnej oparta jest na badaniu klinicznym i histologicznym wykwitów skórnych oraz niewystępowaniu kryteriów świadczących o mastocytozie układowej:

  • typowa wysypka, możliwe występowanie tzw. objawu Dariera
  • pozytywny wynik biopsji skóry:
    • nacieki mastocytowe (> 15 mastocytów) lub rozproszone mastocyty (> 20 mastocytów)
    • lub wykrycie mutacji KIT

2. Mastocytoza układowa (SM) jest chorobą hematologiczną

Mastocytoza układowa jest nowotworem mieloproliferacyjnym. Rozrost mastocytów następuje przynajmniej w jednym z organów (zwykle w szpiku kostnym) i musi zostać potwierdzony przez biopsję. W tym przypadku skóra może, ale nie musi być zaatakowana. Mastocytoza układowa zwykle występuje u dorosłych. Ma ona przebieg chroniczny i jest nieuleczalna. Rokowania w przypadku mastocytozy układowej są zwykle korzystne. Rzadko jedna postać mastocytozy układowej przechodzi w inną. W większości przypadków mastocytoza układowa nie jest chorobą dziedziczną.

a) Różne typy mastocytozy układowej

Mastocytoza układowa o powolnym przebiegu (Indolent Systemie Mastocytosis, ISM): najczęściej spotykana. Może pozostawać w niezmienionej formie przez całe życie pacjenta i rzadko przechodzi w inną postać. Jednym z podtypów tej choroby jest mastocytoza szpiku kostnego, w przypadku której nigdy nie występują zmiany skórne. Innym podtypem jest tląca się mastocytoza układowa, która wykazuje wyższą ilość mastocytów.

Mastocytoza układowa z klonalnym rozrostem linii komórkowych nie- mastocytarnych (SM with Associated Clonal Hematologic Non-Mast Cell-Disease SM-AHNMD): u pacjentów mogą rozwinąć się różne rodzaje chorób krwi. Są one wykrywane w trakcie rutynowych badań krwi lub podczas biopsji szpiku.

Agresywna mastocytoza układowa (Aggressive Systemic Mastocytosis, ASM): chorobie tej towarzyszy zaburzenie funkcjonowania przynajmniej jednego narządu. Rzadko występują zmiany skórne.

Białaczka mastocytarna (Mast Cell Leukaemia, MCL): jest wyjątkowo rzadką chorobą hematologiczną. Białaczce mastocytarnej nigdy nie towarzyszą wykwity skórne.

Istnieją dwie różne i bardzo rzadkie formy nowotworu mastocytarnego. Mięsak mastocytarny (Mast Cell Sarcoma), który jest nowotworem złośliwym oraz Mastocytoma w narządach poza skórą (Extracutaneous Mastocytoma), która jest nowotworem łagodnym i może się znajdować w różnych narządach, ale nie w skórze.

b) Kryteria diagnostyczne mastocytozy układowej

Diagnozę mastocytozy układowej określa się u pacjentów, u których biopsja organów wewnętrznych (zwykle szpiku kostnego) spełnia przynajmniej jedno z głównych oraz przynajmniej trzy poboczne kryteria.

Kryteria główne

Wieloogniskowe gęste nacieki mastocytów (> 15 mastocytów na jednostkę).

Kryteria poboczne

  1. ponad 25% mastocytów świadczy o nieprawidłowej morfologii (wrzecionowaty lub wydłużony kształt)
  2. wykrycie mutacji KIT(receptora o aktywności kinazy tyrozynowej) w kodonie 816
  3. ekspresja CD2 (cluster of differentiation – antygen różnicowania komórkowego) i/lub CD25 w mastocytach
  4. stężenie tryptazy > 20ng/ml

c) Dalsze testy mogą obejmować: morfologię krwi, ultradźwięki, badanie DEXA (dwuwiązkową absorpcjometrię rentgenowską) oraz endoskopię.

Leczenie

Leczenie mastocytozy układowej o powolnym przebiegu opiera się na leczeniu objawów i unikaniu czynników wywołujących degranulację mastocytów. Leczenie polega na podawaniu leków blokujących receptory histaminowe. W przypadku bardziej agresywnych postaci mastocytozy układowej stosuje się leczenie cytoredukcyjne (chemioterapię). W rzadkich przypadkach stosuje się polichemioterapię, promieniowanie lub przeszczep komórek macierzystych.

Mastócitos

Os mastócitos fazem parte do nosso sistema imunológico. Eles são produzidos na medula óssea e de lá passam para outros tecidos onde residem perto dos capilares sanguíneos ou dos nervos. Normalmente, os mastócitos não são encontrados no sangue periférico; portanto, a presença de mastócitos é determinada exclusivamente por uma biópsia tecidual (por exemplo, pele ou medula óssea) e não em amostras de sangue. Os mastócitos produzem e armazenam substâncias (por exemplo, histamina), que podem ser liberadas e, como consequência, produzem sintomas.

Mastocitose

Como indicado, os mastócitos são parte do sistema imunológico de qualquer pessoa. A diferença em pacientes com mastocitose é que em alguns pacientes no qual eles produzem um crescimento anormal e um acúmulo de mastócitos em um ou mais órgãos (por exemplo, pele, medula, fígado, baço), sendo chamado de mastocitose sistêmica.  Ou podem ter uma ativação dos mastócitos, sem ter um crescimento anormal ou acúmulo de mastócitos nos tecidos.  Nessa forma chama-se de síndrome de ativação de mastócitos.  Os sintomas são por iguais em ambos os pacientes.  A mastocitose sistêmica é uma doença que pode ocorrer de muitas maneiras diferentes.

Sintomas

Os sintomas são produzidos, em geral, pela degranulação ou liberação de substâncias (grânulos) armazenadas nos mastócitos. Os mastócitos contêm grânulos com substâncias químicas, como a histamina, que podem ser liberadas fora do mastócito. Esta libertação de mediadores a partir de mastócitos pode conduzir a sintomas tais como vermelhidão, prurido, diarreia, indigestão, náuseas, dor abdominal, muscular / dor óssea, osteoporose, alterações na pressão sanguínea (normalmente hipotensão mas hipertensão pode ocorrer), anafilaxia, ritmo cardíaco elevado, dores de cabeça, problemas de concentração, fadiga, tontura, irritação, raiva, depressão e medo. Esses sintomas podem ser agudos ou crônicos. A gravidade dos sintomas pode ser leve ou envolver risco de vida, embora essa situação seja muito rara e possa depender dos fatores desencadeantes que podem variar muito de um paciente para outro.   No caso raro de mastocitose sistêmica agressiva e na leucemia de mastócitos, os sintomas surgem devido à alteração e destruição de órgãos através da infiltração de mastócitos.

Mastocitose cutânea (MC)

  • Mastocitose maculopapular cutânea (MPCM) [anteriormente chamada de uriticaria pigmentosa (UP)]
  • Mastocitose cutânea difusa (MCD)
  • Mastocitoma

Mastocitose Sistêmica (MS)

  • Mastocitose Sistêmica Indolente (MSI), (a forma mais comum de Mastocitose istêmica)
    • Mastocitose da Medula Óssea
    • Mastocitose Sistêmica Fumegante (MSS)
  • Mastocitose sistêmica com doença hematológica clonal não associada à linha de mastócitos (SM-AHNMD)

A Organização Mundial da Saúde classificou a Mastocitose em duas classes diferentes: Mastocitose Cutânea e Mastocitose Sistêmica.

1. Mastocitose Cutânea (MC), que é uma doença exclusivamente dermatológica, embora os sintomas dos mediadores possam ser sistêmicos.

Nos mastócitos cutâneos, o acúmulo de mastócitos ocorre exclusivamente na pele. É sempre uma doença benigna. A mastocitose cutânea ocorre principalmente em crianças e desaparece em muitas crianças ao redor da puberdade. Na maioria dos casos, a mastocitose cutânea não é hereditária.

a) Diferentes tipos de Mastocitose Cutânea

Mastocitose Cutânea Maculopapular (MCMP) [antiga: Urticária Pigmentosa (UP)] é o padrão mais comum. É caracterizada pela presença de marcas ou bolhas cor-de-rosa ou marrons no corpo, geralmente não no rosto.

Telangiectasia Macularis Eruptiva Perstans (TMEP) é uma forma rara de MCMP. Mostra se como vasos sanguínios finos que coçam e persistem no corpo dos adultos.

Mastocitose Cutânea Difusa (DCM): Forma muito rara da doença que aparece no nascimento e na qual a pele fica espessa e facilmente formada por bolhas.

Mastocitoma: é infrequente, e observado na infância. Pode ser apresentado como um nódulo isolado ou múltiplos.

b) Critérios Diagnósticos de Mastocitose Cutânea

O diagnóstico de mastocitose cutânea baseia-se na clínica e histologia das lesões cutâneas e na ausência de um critério definitivo de envolvimento sistêmico.

  • Exantema típica, o sinal de Darier pode ser positivo.
  • Biópsia de pele positiva:
    • infiltrados de mastócitos (> 15 mastócitos) ou mastócitos dispersos (> 20 mastócitos)
    • – ou detecção da mutação KIT

2. Matocitose Sistêmica (MS) é uma doença hematológica

Mastocitose Sistêmica em Doença Mieloproliferativa. A proliferação de mastócitos está localizada em pelo menos um órgão (geralmente a medula óssea) e deve ser confirmada por biópsia. Pode haver envolvimento da pele – mas não precisa estar presente. A mastocitose sistêmica geralmente ocorre em adultos. É uma doença crônica e até hoje não há cura. O prognóstico da mastocitose sistêmica é geralmente favorável. Raramente uma forma de SM progride para outra. Na maioria dos casos, a EM não é hereditária.

a) Diferentes tipos de Mastocitose Sistêmica

Mastocitose Sistêmica Indolente (MSI): é o padrão mais comum. Pode permanecer sem qualquer mudança para a vida e raramente progride para outro tipo. Um subtipo é Mastocitose da Medula Óssea, que nunca é acompanhada de envolvimento da pele. Outro subgrupo é a Mastocitose Sistémica Fumerante, que apresenta um fardo mastocitário mais elevado.

Mastocitose Sistêmica associada a uma doença do sangue. (SM-AHNMD): vários tipos de desordens sanguíneos podem ocorrer em alguns pacientes. Eles são diagnosticados por um teste de rotina de sangue ou por biópsia da medula óssea.

Mastocitose Sistêmica Agressiva (MSA): esta doença apresenta o mau funcionamento de pelo menos um órgão. O envolvimento da pele é raro.

Leucemia Mastocitária (LM): é uma doença hematológica excepcionalmente rara. Lesões de pele nunca estão presentes.

b) Critérios Diagnósticos em Mastocitose Sistêmica

O diagnóstico de mastocitose sistêmica é estabelecido em pacientes cuja biópsia de um órgão interno (geralmente medula óssea) satisfaz pelo menos um critério maior e um menor, ou por três critérios menores.

Critério Maior

Infiltrados multifocais compactos de mastócitos (> 15 mastócitos por agregado).

Critérios Menor

Mais de 25% dos mastócitos mostram uma morfologia atípica dos mastócitos (em forma de fuso (spindle shaped) ou alongada)

Presença da mutação KIT no códon 816

Os mastócitos expressam CD2 e / ou CD25

Níveis séricos de triptase > 20ng / mL

c) Outros testes diagnósticos podem incluir: hemograma completa, ultrasom, escan DEXA, endoscopia & colonoscopia com exame imunohistoquimicos com marcadores para mastócitos.

Tratamento

O tratamento das formas indolentes dependem dos sintomas e consiste em evitar os fatores desencadeantes que podem causar a degranulação dos mastócitos. Parte do tratamento é baseada em antagonistas do receptor de histamina, estabelizadores de mastócitos, inibidores de leucotrines e prostaglandinas, entre outros mediadores.  As formas mais agressivas de mastocitose sistêmica são tratados por remédios citoredutores  específicos.  Raramente poliquimioterapia, radiação ou transplante de células-tronco podem ser necessários.

Mastocitele

In mod normal mastocitele nu se găsesc în sânge periferic; drept pentru care, prezenţa mastocitelor se determină exclusiv prin intermediul unei biopsii de ţesut (de exemplu piele sau măduvă) şi nu în mostre de sânge. Mastocitele produc şi depozitează substanţe (de exemplu histamina), care se pot elibera şi ca urmare vor produce simptome.

Mastocitozele

Cum s-a indicat, mastocitele sunt parte a sistemului imunitar al oricărei persoane. Diferenţa la pacienţii cu mastocitoză este că la ei se produce o creştere anormală şi o acumulare de mastocite în unul sau mai multe organe (ex. piele, măduvă, ficat, splină). Mastocitozele sunt boli care se pot prezenta în forme foarte diverse.

Simptome

Simptomele sunt cauzate, în general, de degranularea sau eliberarea de substanţe (granule) depozitate în mastocite. Mastocitele conţin granule cu substanţe chimice, cum ar fi histamina, care se pot elibera în exteriorul mastocitului. Această eliberare a mediatorilor mastocitelor poate conduce la simptome cum ar fi înroşire, prurit, diaree, indigestie, greaţă, durere abdominală, durere musculară/osoasă, osteoporoză, schimbări în presiunea sangvină(în mod normal hipotensiune), anafilaxii, frecvenţe cardiace înalte, dureri de cap, probleme de concentrare, oboseală, ameţeli, iritare, supărare, depresie şi frică. Aceste simptome pot fi acute sau cronice. Severitatea simptomelor poate fi uşoară sau să ducă la risc vital, chiar dacă această situaţie este foarte puţin frecventă, şi poate depinde de factorii declanşatori care pot varia mult de la un pacient la altul.

Organizaţia Mondială a Sănătăţii a clasificat mastocitozele în două clase diferite: Mastocitoze cutanate şi Mastocitoze sistemice.

1. Mastocitoze Cutanate (MC)care sunt o boală exclusiv dermatologică, chiar dacă simptomele mediatorilor pot fi sistemice.

În mastocitozele cutanate acumularea de mastocite se produce în piele în mod exclusiv. Este ÎNTOTDEAUNA o boală benignă. Mastocitoza Cutanată se produce în principal la copii şi în cazul multor copii dispare în jurul perioadei de pubertate. În majoritatea cazurilor mastocitoza cutanată nu este ereditară.

a) Diferitele tipuri de mastocitoză cutanată

Mastocitoză Cutanată Maculopapulară (MCMP) [anterior Urticaria Pigmentosa (UP)] este varianta cea mai comună. Se caracterizează prin prezenţa de pete roz sau maro şi/sau vezicule pe corp, în mod normal nu pe faţă. Telangiectasia macularis eruptiva perstans (TMEP) este o formă rară de MCMP. Prezintă o umflătură mare roz persistentă pe corpul adulţilor.

Mastocitoza Cutanată Difuză (MCD): O formă foarte rară a bolii care apare la naştere şi în care pielea este îngroşată şi formează vezicule foarte uşor.

Mastocitom: puţin frecvent, se observă în copilărie. Se poate prezenta ca un nodul izolat sau mai mulţi.

b) Criterii diagnosticare mastocitoză cutanată

Diagnosticul mastocitozei cutanate se bazează pe partea clinică şi histologia leziunilor cutanate şi absenţa unui criteriu definitiv de implicare sistemică.

  • Biopsia de piele pozitivă:
    • infiltraţii de mastocite (> 15 mastocite) sau mastocite dispersate (> 20 mastocite)
    • – sau detectarea mutaţiei KIT

2. Matocitoza Sistemică (MS) este o boală hematologică

Mastocitoza Sistemică într-o boală mieloproliferativă. Proliferarea mastocitelor se localizează în cel puţin un organ ( în mod normal în măduva osoasă) şi trebuie să se confirme cu biopsia. Poate exista implicarea pielii – dar nu e necesar să fie prezentă. Mastocitoza sistemică apare în mod normal la adulţi. Este o boală cronică şi în prezent nu există tratament. Prognosticul mastocitozei sistemice este în mod normal favorabil. Rareori o formă de MS poate progresa către o alta. În majoritatea cazurilor MS no sunt ereditare.

a) Diferitele Tipuri de Mastocitoză Sistemică

Mastocitoza indolentă Sistemică (MIS): este varianta cea mai comună. Poate ramâne fără nicio schimbare pentru toată viaţa şi rareori progresează către alt tip. Un subtip este Mastocitoza de Măduvă, care niciodată nu este însoţită de implicarea pielii. Alt subgrup este Mastocitoza Sistemică Latentă, care prezintă o mare încărcare de mastocite.

MS asociată cu o alterare a sângelui (MS-AHNMD): la unii pacienţi se pot prezenta diferite tipuri de perturbări în sânge. Sunt diagnosticate printr-un test de rutină. Sunt localizate printr-un test în sânge sau printr-o biopsie de măduvă.

Mastocitoza Sistemică Agresivă (MSA): această boală prezintă proasta funcţionare a cel puţin unui organ. Implicarea pielii este rară.

Leucemia Mastocitului (MCL): Aceasta este o boală hematologică în mod excepţional rară. Leziunile cutanate nu sunt niciodată prezente.

b) Criterii Diagnosticare Mastocitoze sistemice

Diagnosticul de Mastocitoză Sistemică se stabileşte la pacienţii a căror biopsie a unui organ intern (în general măduva osoasă) îndeplineşte cel puţin un criteriu major şi unul minor, sau cel puţin trei criterii.

Criteriu Major

Infiltraţii multifocale compacte de mastocite (> 15 mastocite per agregat).

Criterii Minore

  1. Mai mult de 25% dintre mastocite prezintă o morfologie atipică a mastocitului (cu formă de fus sau elongaţii)
  2. Prezenţa mutaţiei de KIT în codarul 816
  3. Mastocitele exprimă CD2 şi/sau CD25
  4. Nivele de triptază serică mai mari de 20ng/mL

c) Alte probe de diagnostic pot include: Calcul atent total de sânge DEX. Scanare Ultrasunete DEXA şi endoscopii.

Tratament

Tratamentul formelor indolente ale bolii depinde de simptome şi constă în evitarea factorilor declanşatori care pot cauza degranularea mastocitului. Parte din tratament se bazează pe antagonişti ai receptorilor de histamină.

Mastociti

Mastociti so celice, ki so del imunskega sistema. Proizvajajo se v kostnem mozgu in potujejo do tarčnih tkiv, kjer se nahajajo v bližini krvnih žil ali živcev. Po navadi se mastociti ne nahajajo v periferni krvi, zato se prisotnost mastocitov ugotavlja zgolj z biopsijo tkiv (npr. kožna biopsija ali biopsija kostnega mozga) in ne z odvzemom krvi. Mastociti proizvajajo in shranjujejo različne biološko aktivne snovi (npr. histamin), ki se lahko sproščajo in povzročajo različne simptome.

Mastocitoza

Mastociti kot sestavni del imunskega sistema so prisotni pri vsakem posamezniku. Razlika je le v tem, da je količina mastocitov pri bolnikih z mastocitozo izjemno povečana in da se le ti kopičijo v enem ali več organih (npr. koža, kostni mozeg, jetra, vranica). Mastocitoza se pojavlja v več različnih oblikah.

Simptomi

Simptome ponavadi povzroča degranulacija oz sproščanje snovi (granul) iz mastocitov.

Mastociti vsebujejo skupke (granule) biološko aktivnih snovi, predvsem histamine, ki se lahko sproščajo v okolico mastocitov. Le to lahko vodi do različnih simptomov, kot so rdečica (flushing), srbečica (pruritus), driska (diareja), slabša prebava, slabost, bolečine v trebuhu, bolečine v mišicah, kosteh, osteoporoza, spremembe v krvnem pritisku (hipotenzijo oz. znižanje krvnega tlaka), anafilaktični šok, hiter utrip srca, glavobol, težave s koncentracijo, utrujenost, zamegljen vid, vrtoglavico, razdražljivost, jezo, depresijo in strah. Pri vsaki obliki mastocitoze so simptomi lahko akutni ali kronični. Simptomi so lahko blagi ali življenjsko nevarni, vendar se le-ti pojavljaj zelo redko in so odvisni od dražljajev, ki se med bolniki zelo razlikujejo.

Svetovna zdravstvena organizacija je razdelila mastocitozo v dve skupini: kožna mastocitoza in sistemska mastocitoza.

1. Kožna mastocitozaje izključno dermatološka bolezen, čeprav so simptomi lahko tudi sistemski.

Pri kožni mastocitozi se mastociti kopičijo izključno v ali na koži. Je VEDNO benigna bolezen. Kožna mastocitoza se večinoma pojavlja pri otrocih in v večini primerov izzveni okoli pubertete in ni dedna.

a) Različne vrste kožne mastocitoze

  • makulopapularna oblika (nekoč Urticaria Pigmentosa) je najbolj pogosta oblika kožne mastocitoze. Po telesu je vidno veliko majhnih rožnatih ali rjavkastih madežev ali oteklin, vendar ne po obrazu. Telangiectasia macularis eruptiva perstans je redka podvrstamakulopapularne kožne mastocitoze. Pojavlja se pri odraslih. Za to obliko so značilni vztrajajoči rožnati madeži kože, prisotna je rdečica in srbečica, vidne so tudi fine krvne žilice.
  • difuzna kožna mastocitoza: je zelo redka oblika, prisotna že od rojstva. Koža je zadebeljena, zlahka se pojavljajo mehurji.
  • mastocitom kože: je redka oblika, prisotna pri dojenčkih. Za njo so značilni posamezni ali številni privzdignjeni vozlički.

b) Diagnostična merila kožne mastocitoze

Bolezen odkrijemo s histološkim pregledom odvzetega tkiva, najpogosteje kožne spremembe in kostnega mozga in z izključitvijo definitivnih kriterijev sistemsko mastocitoze:

  • Tipični eksantem, pozitiven Darierjev znak
  • Pozitivna biopsija kože:
    • infiltrati mastocitov (> 15 mastocitov) ali razpršeni mastociti (> 20 mastocitov)
    • Prisotnost mutacije na KITkinazi

2. Sistemska mastocitoza (SM) je hematološka bolezen

Sistemska mastocitoza je mieloproliferativna neoplazma. Nenadzorovana delitev mastocitov se nahaja v najmanj enem od notranjih organov (po navadi v kostnem mozgu) in mora biti potrjena z biopsijo kože. Izpuščaji na koži so lahko prisotni, ni pa nujno. Sistemska mastocitoza se pojavi večinoma pri odraslih. Ima kroničen potek in ni ozdravljiva. Prognoza sistemske mastocitoze je v večini primerov dobra. Redko ena oblika sistemske mastocitoze preide v drugo. V večini primerov, sistemska mastocitoza ni dedna.

a) različne vrste sistemske mastocitoze

Indolentna sistemska mastocitota je najbolj pogosta oblika SM. Lahko ostane nespremenjena skozi celo življenje in redko preide v drugo vrsto SM. Ena izmed podvrst SM je mastocitoza kostnega mozga, kjer koža ni nikoli prizadeta. Druga podvrsta je prikrita (sam ne vem če je prav) sistemska mastocitoza, kjer je število mastocitov izjemno povečano.

SM s pridruženo ne – mastocitno hematološko klonalno boleznijo: pri manjšem številu bolnikov z SM se lahko razvije več različnih hematoloških bolezni. Odkrijejo se z rutinskim odvzemom krvi ali biopsijo kostnega mozga.

Agresivna sistemska mastocitoza: bolezen spremlja prizadetost vsaj enega notranjega organa, prizadetost kože je redka.

Mastocitna levkemija: je izjemno redka oblika hematološke bolezni, kjer koža ni nikoli prizadeta.

b) Diagnostična merila sistemske mastocitoze

Bolezen je prisotna če biopsija notranjega organa (po navadi kostnega mozga) izpolnjuje glavno in eno ozmed pomožnih meril ali vsaj tri pomožna merila.

Glavno merilo:

Multifokalni kompaktni infiltrati mastocitov v kostnem mozgu ali drugem organu (>15 MS)

Pomožna merila

  1. Mastociti s patološko vretenasto obliko (> 25%)
  2. Prisotnost mutacije na KITkinazi
  3. Mastociti s patološkim izražanjem CD2 in/ali CD25
  4. Serumska triptaza > 20 ng/ml

c) Nadaljnji testi lahko vključujejo: krvna slika, ultrazvok, DEXA skeniranje in endoskopije.

Zdravljenje

Zdravljenje indolentne sistemske mastocitoze je odvisno od simptomov in predvsem od izogibanja posameznim povzročiteljem degranulacije mastocitov. Zdravljenje temelji na blokatorjih histaminskih receptorjev.

Mastocitos

Los mastocitos son parte de nuestro sistema immune. Se producen en la médula ósea y desde ahí pasan a otros tejidos donde residen cerca de los capilares sanguineos o de los nervios. Normalmente los mastocitos no se encuentran en sangre periférica; por lo tanto, la presencia de mastocitos se determina exclusivamente mediante una biopsia de tejido (por ejemplo piel o médula) y no en muestras de sangre. Los mastocitos producen y almacenan sustancias (por ej. histamina), que puede liberarse y como consecuencia producirá síntomas.

Mastocitosis

Como se ha indicado, los mastocitos son parte del sistema inmune de cualquier persona. La diferencia en los pacientes con mastocitosis es que en ellos se produce un crecimiento anormal y una acumulación de mastocitos en uno o más órganos (ej. piel, médula, hígado, bazo). Las mastocitosis son una enfermedad que se puede presentar de formas muy diversas.

Síntomas

Los síntomas se producen, en general, por la degranulación o liberación de sustancias (gránulos) almacenadas en los mastocitos. Los mastocitos contienen gránulos con sustancias químicas, como la histamina, que se pueden liberar al exterior del mastocito. Esta liberación de los mediadores de los mastocitos puede conducir a síntomas tales como enrojecimiento, prurito, diarrea, indigestión, nauseas, dolor abdominal, dolor muscular/óseo, osteoporosis, cambios en la presión sanguínea (normalmente hipotensión), anafilaxias, frecuencia cardiacas altas, dolores de cabeza, problemas de concentración, fatiga, mareos, irritación, enfado, depresión y miedo. Estos síntomas pueden ser agudos o crónicos. La severidad de los síntomas puede ser suave o conllevar riesgo vital, aunque esta situación es muy infrecuente, y puede depender de los factores desencadenantes que pueden variar mucho de un paciente a otro.

La Organización Mundial de la Salud ha clasificado las mastocitosis en dos clases diferentes: Mastocitosis cutáneas y Mastocitosis sistémicas.

1. Mastocitosis Cutáneas (MC), que son una enfermedad exclusivamente dermatológica, aunque los síntomas de los mediadores pueden ser sistémicos.

En las mastocitois cutáneas la acumulación de mastocitos se produce en la piel exclusivamente. Es SIEMPRE una enfermedad benigna. La mastocitosis Cutánea se produce principalmente en niños y en muchos niños desaparece en torno a la pubertad. En la mayoría de los casos la mastocitosis cutánea no es hereditaria.

a) Diferentes tipos de mastocitosis cutanea

Mastocitosis Cutanea Maculopapular (MCMP) [anteriormente Urticaria Pigmentosa (UP)] es el patrón más común. Se caracteriza por la presencia de marcas rosas o marrones y/o ampollas en el cuerpo, normalmente no en la cara. Telangiectasia macularis eruptiva perstans (TMEP) es una forma rara de MCMP. Muestra un ronchón rosa persistente en el cuerpo de los adultos.

Mastocitosis Cutánea Difusa (DCM): Una forma muy rara de la enfermedad que aparece en el nacimiento y en la que la piel esta engrosada y forma ampollas fácilmente.

Mastocitoma: poco frecuente, se observa en la infancia. Se puede presentar como un nodulo aislado o multiples.

b) Criterios diagnósticos de mastocitosis cutanea

El diagnóstico de la mastocitosis cutánea se basa en la clínica e histología de las lesiones cutáneas y la ausencia de un criterio definitivo de implicación sistémica.

  • Exantema típico, el signo de Darier puede ser positivo.
  • Biopsia de piel positiva:
    • infiltrados de mastocitos (> 15 mastocitos) o mastocitos dispersos (> 20 mastocitos)
    • – o detección de la mutación de KIT

2. Matocitosis Sistémica (SM) es una enfermedad hematológica

La Mastocitosis Sistémicas en una enfermedad mieloproliferativa. La proliferanción de los mastocitos se localiza en al menos un órgano (normalmente la médula ósea) y debe confirmarse con biopsia. Puede haber implicación de la piel – pero no tiene porque estar presente. La mastocitosis sistémicas normalmente se da en adultos. Es una enfermedad crónica y a día de hoy no hay cura. El pronóstico de las mastocitosis sistémica es normalmente favorable. Raramente una forma de SM progresa a otra. En la mayoría de los casos las MS no son hereditarias.

a) Diferentes Tipos de Mastocitosis Sistémica

Mastocitosis indolente Sistématica (MIS): es el patrón más común. Puede permanecer sin ningún cambio de por vida y raramente progresa a otro tipo. Un subtipo es la Mastocitosis es la Mastocitosis de Médula, que nunca va acompañada de implicación de la piel. Otro subgrupo es Mastocitosis Sistémica Latente, que presenta una alta carga de mastocitos.

MS asociada con una alteración de la sangre. (SM-AHNMD): en algunos pacientes se pueden presentar varios tipos de desajustes en sangre. Se les diagnostica por un test de rutina. Se les localiza por un test en Sangre o por biopsia de médula.

Mastocitosis Sistémica Agresiva (MSA): esta enfermedad presenta el malfuncionamiento de al menos un órgano. La implicación de la piel es rara.

Leucemia del Mastocito (MCL): esta es una enfermedad hematológica excepcionalmente rara. Las lesiones cutáneas nunca están presentes.

b) Criterios Diagnóstics en Mastocitosis sistémicas

El diagnóstico de Mastocitosis Sistémica se establece en pacientes cuya biopsia de un órgano interno (en general medula ósea) cumpla al menos un criterio mayor y uno menor, o al menos tres criterios.

Criterio Mayor

Infiltrados multifocales compactos de mastocitos (> 15 mastocitos por agregado).

Criterios Menores

  1. Más del 25%de los mastocitos muestran una morfología atípica del mastocito (con forma de huso o elongados)
  2. Presencia de la mutación de KITen el codon 816
  3. Los mastocitos expresan CD2 y/o CD25
  4. Niveles de triptasa sérica mayors de 20ng/mL

c) Otras pruebas diagnóstica pueden inclur: recuento total de sangre DEX. Ultra sonidos scan DEXA y endoscopies.

Tratamiento

El tratamiento de las formas indolentes de la enfermedad depende de los síntomas y consiste en evitar los factores desencadenanates que pueden causar desgranulación del mastocito. Parte del tratamiento se basa en antagonistas de los receptores de histamina.
